administer medications, treatments and procedures, develop and update care
plans and work directly with the physician to meet the needs of the

perform fundamental nursing tasks such as vital signs, personal hygiene,

| Formulates individualized care plans based on
initial evaluation, in conjunction with the assigned home care
professional, patient and/or family member. Plans and updates the patient
progress and places emphasis on education and training of the patient and

Social Worker |

Assist the health care staff, patient and family
in understanding the significant social and emotional factors related to
the patients health problems. Helps the patient cope with the problems
of daily living. |
